Heartfelt is a set of cards and a transplant guide designed to support open conversations about heart transplantation. It combines education with game elements to help young patients, their families, and peers understand the process and emotions involved. Created in collaboration with patients and the Silesian Center for Heart Disease, the project fosters empathy, breaks communication barriers, and supports teachers, psychologists, and medical staff in building an open, understanding environment.
